Level 131 takes the form of a distorted hospital with its typical green environment.

Description

Level 131 is a hospital with infinite floors and finite size that measures about 550×550m² from corner to corner. The hallways are also non-Euclidean along with their effect of being non-linear. Every hallway, spacious room and corner is illuminated by a gloomy green neon light, which comes mostly from the exit signs that exist, although hallways with fluorescent lights that give off this green color have also been seen. The floor is smooth, similar to polished stone. The walls can be made of concrete or bricks or smooth stone as well as the floor. The ceiling is made of marble with circular LED lights turned off or even broken in some sections. All of the hallways on Level 131 are decorated with doors that lead to different typical hospital rooms, but in a much more disorganized state and without safe supplies.

The different rooms within Level 131 can resemble an X-ray room, an operating room, typical medical rooms, among other types. These rooms are devoid of supplies and you can only find broken blood bags and oxygenated blood bags, dirty beds, shattered glass, useless devices that sometimes explode, etc. The doors have different labels with strange symbols arranged in an unknown way, they mostly use numbers along with Greek symbols, but there are some exceptions where they use letters, questions, exclamations, Spanish, Japanese money symbols, etc, and many more combinations that are not mentioned have been able to decipher so far. Even though there is a theory that Level 131 was inhabited by unknown entities that used that type of format to be able to communicate or name a room.
