Description
Level 415 is an abandoned hospital with an extremely harsh environment. The three storey structure's interior and staircases have a degree of mold and decay that suggests it has been abandoned for decades. The afformentioned staircases can be found at the end of each floor's hallway, rotting away like the rest of the hospital. The halls are covered with trash and debris, which can obstruct a wanderer's movement. Scalpels and scissors can be found strewn across the sprawling halls, posing as a safety hazard to wanderers due to their incredibly sharp nature. All floors include 20 hospital rooms, and each hospital room includes one bed and a desk. These rooms pose no threat other than being cut by sharp objects. Some personal objects, such as frames, photos, and broken computers, can be found in a few rooms as well. All of the hospital rooms remain unlocked at all times, with the exception of room 315. Since approaching Room 315 can cause psychological problems, only some information was reported. According to the patient card attached to the door of Room 315, it accommodates a patient named Jane. Along with this, screaming presumed to be a girl's voice can be heard near Room 315, which is theorized to cause the numerous psychological effects associated with Room 315. Outwardly, each floor does not have much difference from one another, but the third floor is more dangerous since it includes Room 315 and more sharp objects than other floors.
A hospital room
Level 415 does not have any semblance of a day-night cycle, maintaining nightime regardless of the hour. Furthermore, any external light sources have proven to malfunction and not work due to unknown reasons. Despite this, there is still a presiding unknown source of light that can be found across the level. Due to the afformentioned inability to use light sources or machinery of any sort, there have proven to be very few photos obtained from this level; any form of camera having reported to have been damaged upon entering, owing to the level's air particle concentration. The atmosphere in level 415 maintains a temperature of 10ºC - 13ºC; lingering in the air is a pungent odour and ominous scent, bringing discomfort to wanderers. In the hallways, there are windows arranged at regular intervals, which only show darkness. All tries to open or break the windows have failed. Since windows only show darkness, one cannot see the outside of hospital.
When wanderers enter Level 415, they feel guilty over their past actions and a sense of loss. The longer that wanderers stay in Level 415, the stronger this sense becomes. This sense confuses wanderers and disturbs their movement, causing them to lose their sense of direction and have blurry vision. A disoriented wanderer will move according to their gut feeling, which can lead to getting cut. Drinking Almond Water does not help with this effect.
It is crucial to leave Level 415 as soon as possible, due to danger caused by phenomena and objects as well as the level having a harsh environment, as mentioned above. Sometimes many, or all things in the level, are broken up into flying debris. Windows are shattered, leaving pieces of glass that will fly towards the wanderer. Frames and any other objects including paper, will burst when the wanderer approaches it. Even the slightest contact with the flying objects can cause harm to the wanderer's body. Wearing clothing does not prevent this, and even a small cut1 can cause serious problems. It is advised that wanderers avoid as many objects as possible on this level, due to the dangerous phenomenon, Burning Pain.
Wanderers have reported difficulty breathing in this level due to a high amount of dust in the air, and since dust doesn't make smoke or fog, It is difficult to determine the extent of the dust, but dust can be prevented by wearing proper protective gear. Certain rooms such as surveillance rooms, reference rooms, and counseling offices are placed randomly on each of the floors. These rooms have similar dangers to the other areas on this level, and the objects in this level should not be studied. It is advised to explore this level with caution and to avoid all objects and obstacles.
Phenomena
Level 415 has two kinds of phenomena, which makes the level significantly more dangerous.
Burning Pain
If a wanderer makes contact with any object in Level 415, they will be cut in that area and it will start to bleed. The length and depth of the cut varies by the amount of contact made with the object. Immediately after the cut starts bleeding, wanderers will feel a burning sensation. This sensation starts in the area of the cut, and spreads throughout the entire body during a 3 minute time period. As this occurs, the amount of pain the wanderer feels will increase, eventually leading to their death. Witnesses have reported seeing severe burns covering the bodies.
To avoid this, follow these instructions:
1. If you decide to explore Level 415, wear thick and protective clothes. While clothes can’t protect you from the objects, it can minimize the effects and prevent minor wounds like paper cuts.
2. Do not treat the wound with Almond Water, as this does not help and only makes it worse.
3. When you get hurt from something, you can prevent spread of pain by cutting off the wounded area. You have to cut it off as soon as possible to prevent further damage. The wound will decompose more and more, and it’ll start to swell. The wound will increase in size as this happens. The cut-off section will be exposed to dust and cause serious problems, so it is recommended to cover the exposed section with an external object2 for safety.
Touching the body decayed and burnt by this phenomenon will also harm the wanderer, so ignoring dead body is recommended. The body will soon begin to decompose, and after a few minutes, it has been reported to be completely decomposed and generating dust. Dead wanderer's belongings won't be decomposed, but one shouldn't take the materials since dead wanderer's belongings are treated as Level 415 garbage after wanderer's body has been completely decomposed.
Flashbacks
If a wanderer is careful enough in the surrounding environment, the first phenomenon can be prevented. However, during the Flashback phenomenon, wanderers will start to feel a guilty conscience about their past memories as soon as they enter Level 415. This includes not only misdemeanors, but also minor actions, such as littering, playing a light prank on a friend, or stepping on ants. The wanderer feels a heavy regret for their actions and will start to feel a strong sense of distress. As mentioned before, this causes a loss of direction, which will lead to getting hurt. Unlike the first phenomenon, Flashbacks cannot be prevented.
Seeing the objects in the level including frames, paper, and books triggers this phenomenon, but it also causes a mysterious sense of nostalgia that makes the wanderer approach them. A rescued wanderer, who prefers to stay anonymous, said that approaching these objects felt like repenting for their past. They reported that the feeling became stronger over time, but they were able to overcome it eventually.
I started to feel guilty and regret my past actions. I couldn't shake off that feeling, even though I closed my eyes and covered my ears with a hand. When I saw a can of trash, I started to think I had thrown it away, and I also felt I should take it. As a result, I lost my left arm.
I can't know why I cared about it, and misunderstood that the can was mine.
Thanks to the pain of burning, I could recover my senses, and I had cut my arm to prevent the phenomenon.

Jane
In Room 315, which is locked, it is believed that a patient named Jane Doe resides there. Since Room 315 hasn't been explored yet, no one knows what the room consists of or what happens inside. However, a wanderer reported that Jane seemed to be suffering from some sort of mental or physical distress. On occasion, screaming can be heard in the level. The screaming appears to be sourced from Room 315.
All attempts to open Room 315 have resulted in failure, therefore only assumptions can be made about Room 315 and Jane. Theories suggest that she seems to be suffering from the level phenomenon “Burning Pain”. Wanderers have reported feeling sorry for Jane, but when they approached Room 315, they felt a feeling of misery. Because of this, exploring the third floor for a long time and approaching Jane is never recommended.

There have been claims from a wanderer named Koma, saying that he saw Jane. He said when he approached Room 315, he found the door slightly open. When he looked into the room through a gap, he saw the room was filled with sharp objects. The door was not opened further, and started to close slowly. What he saw before the door closed was a girl seemed to be suffering from a lot of trauma. He said the girl was covered by curtains, so the exact appearance of Jane is not known. While this is highly regarded as true, there is no evidence to support this. Since Jane is still being investigated, it is believed that Jane can't die. Including this fact, serveral reports back up the possibility that Jane is an entity.
Since Jane's personal information couldn't be found, not much is known about Jane. But, according to the rumors, Jane had a crime of kidnapping an unidentified child. There's no record of Jane being sentenced to prison, and that was all she had been ordered off-limits by the child's parents.
That was not a fair punishment.
What's happening in the locked room?
Endless screaming was bothering me, so I couldn't pay attention to the room.
But, I can remember one thing at least.
I don't know why, but all the most dangerous pointed objects seemed to be concentrated in the locked room. Like… they look like to want to punish the strange girl trapped in that room.

Objects
Though most objects in Level 415 are decayed and burnt, making it hard to classify them, some objects in good condition have been reported. In particular, the following objects have been reported considerably more than others:
- Several documents marked for a "Jane Doe". Further investigation indicates that Doe had been submitted to an unknown hospital for lacerations and physical trauma around the neck and face.
- Several children's toys, including dolls, wooden building blocks, and minature cars.
- A 'thank you!' flyer for attendance of a funeral service for one "Margaret Sane(Maggy), age 5."
- Torn news paper head line section, which is dealing with a kidnap incident.
- Court papers near the edge of the bed within Level 415's hospital rooms. It appears to be a restraining order, although the recipient of said order cannot be discerned. Arthur Sane
Entrances And Exits
Entrances
Wanderer can enter level 415 by no-clipping in dark side of some levels.
The levels observed so far are as follows;
For Level 940 and Level 999, wanderers can find areas that are exceptionally dark even when the level is always dark, which leads them to Level 415, causing no-clipping.
Exits
In the third floor, there is a stairwell leading to the rooftop. Wanderers can exit to level 237 by opening the rooftop door, and stepping on the rooftop. It is advised to be cautious when one opens the rooftop door, since the rust on the handle is sharp. Using gloves is recommended. But
